20-30% of men suffer from sexual dysfunction at some point in their lives which may be any of the following: erectile dysfunction (ED) (impotence), premature ejaculation, prostate problems, libido and male fertility (infertility). Sexual dysfunction can have a negative effect on a man’s relationships and enjoyment in life if left untreated.
Erectile dysfunction is very common form of sexual dysfunction and is defined as persistent inability to attain and maintain an erection sufficient to permit satisfactory intercourse.
The most important physical causes of ED are high blood pressure, coronary arterial disease, diabetes, cardiovascular disease and diabetes, neurological problems (for example, trauma from prostatectomy surgery), hormonal insufficiencies (hypogonadism) and drug side effects.
There are various and often multiple underlying causes of ED, some of which are treatable medical conditions. Damage to nerves, arteries, smooth muscles, and fibrous tissues, often as a result of diseases such as diabetes, kidney disease, chronic alcoholism, multiple sclerosis, atherosclerosis, vascular disease, and neurologic disease are the most common causes of ED. Also, surgery (especially radical prostate and bladder surgery for cancer) can injure nerves and arteries near the penis, causing ED. Injury to the penis, spinal cord, prostate, bladder, and pelvis can lead to ED by harming nerves, smooth muscles, arteries, and fibrous tissues of the corpora cavernosa. In addition, many common medicines—blood pressure drugs, antihistamines, antidepressants, tranquilizers, appetite suppressants, and cimetidine (an ulcer drug)—can produce ED as a side effect.
Experts also believe that psychological factors such as stress, anxiety, guilt, depression, low self-esteem, and fear of sexual failure can be a contributing factor of ED. Other possible causes are smoking, which affects blood flow in veins and arteries, and hormonal abnormalities, such as not enough testosterone.
Although it is not clear how they are related, it is thought that sufferers of benign prostatic hyperplasia (BPH), in particular older men, are more likely to suffer from ED.
Penile erection occurs as a result of increased blood inflow to the penis, engorgement with blood, and decreased outflow of blood from the penis.
